1st

Client Intake

Complete and Submit Request for Clinic Registration

Please download, fill in the Health Survey form (see the download section below). You will need to attach the health survey, before submitting your client intake request.



2nd

System Entry

Submit Data for File Making

Upon receipt of your Client Intake Request submitted above, we will review the information. Once it has been approved for “case service”, you will receive an email invitation to register for our Client Portal.
You will then complete the portal registration and upload your ID.
Once we have all the information that we need and have completed the verification process, we will create your file, and approve your access to the portal(you will not be able to login, before we are done).
NOTE: You will be updated once the process is complete, and, you are allowed to login.

3rd

If you have completed STEP-3, allow 48 Hours, then; Kindly Check your email for verification and login instructions.
***Check you spam folder to ensure the email was not sent there. Then Login to your dashboard.
